The normal fee for ordering an application packet by mail is $50. You now have the option to download the forms instead. There is no charge for the download, but a one time $25 administrative fee should be added when submitting the completed application to NARM.

If you are a Registered Midwife (RM) in the United Kingdom (UK), you must fill out and submit the following forms:
Continuity of Care—Practical Experience Form 200
Out-of-Hospital Documentation Form 204
and you must contact the Nursing and Midwifery Council to have them send directly to the NARM Applications Department evidence of current registration as a midwife in the UK.
